Resource Room Catalog for TAP at CTF

• Resource Room Catalog 2010
  The Resource Room Catalog contains a listing of all the books, software, audio-visuals, games, and activity sets we have available for checkout. To check out any item in the catalog, please visit our location during regular business hours at 689 Castle Drive, Charleston, IL.

• Social Skills Form  
  Complete and submit this form if your child is new to The Autism Program at CTF and will be attending our Social Skills Group or Play Group.
  
  
• Visual Aid Request Form
  Complete and submit this form if you would like TAP to design a customized visual aid for an individual with autsim. This could be a calendar, choice board, schedule, or other visual aid. Please note that there may be a nominal fee for some visual aids to cover our cost for materials.

  Complete and submit this form to register as a new member of The Young Adult Club (YAC) of Champaign. Note: Do not use this form if joining YAC in Charleston (use the form listed below).
