Qualified Mental Health Professional (QMHP): South Holland, IL

CTF ILLINOIS is looking for a full-time QMHP for our South Holland location. The QMHP will provide clinical services to chronically mentally ill adults enrolled in psychosocial rehabilitation and community support services. Duties include, but are not limited to, completing assessments, developing treatment plans, community support, individual therapy, group therapy, crisis and client-centered consultation, mental health assessments, development and updating of individual treatment plans (ITPs) every six months or as needed, and other duties as assigned. 

Requirements:

  • Master's degree in Psychology, Social Work, or Counseling is required
  • Must meet eligibility requirements as a QMHP as defined in Medicaid Rule 132
  • Must be able to work up to a 40 hour flexible work week
  • Must be able to transport passengers in a 15-passenger van (leased through PACE)
  • Must have current/valid driver's license with an acceptable driving record and be able to pass and maintain PACE certification

Full-Time Behavioral Health Therapist: Charleston, IL

Behavioral Health Therapist needed to provide individual, group therapy, counseling, and other related mental health services. Development of treatment plans that identify a person’s skills and life needs. Assessment of a person’s individual strengths, needs, services and/or natural supports. Address gaps in services while providing programming and community success.

Duties include: Providing billable therapy and counseling services to external clientele, and completion of comprehensive intake assessments according to DHS and CQL standards on a daily basis. Monitoring the implementation of services identified in the plan, and their responsiveness to the changing needs of the person served. Participation in development of the person-centered plan, as well as providing support strategies in collaboration with the interdisciplinary team. Creating adjustment of the plan according to program requirements and each person’s specific needs.

Requirements:

  • Master's degree (M.A.) or equivalent in Counseling, Psychology, Social Work, or related field and prior internship in counseling
  • Must be a licensed clinical professional counselor (LCPC), a licensed clinical social worker (LCSW), a licensed clinical psychologist, or a licensed marriage and family therapist
  • Basic user knowledge of Microsoft Office (Excel, Word, Outlook,etc.), and/or other PC applications

Full-Time LPN: Charleston, IL

Would you like a nursing position where you have daily contact with the people for whom you are caring? The opportunity to develop ongoing relationships with them?  Without Shift Work?

We have the opportunity you are looking for. CTF ILLINOIS needs a full time LPN to coordinate medical services for individuals with developmental disabilities in local group homes in Charleston. The LPN performs general health assessments, first aid, and ordered treatment. The LPN will work with residents and train staff members on self-administration of medication.

Weekday hours during typical business hours, with limited on-call for phone triage evenings and periodically scheduled weekends. Company phone and mileage reimbursement provided. FT positions are eligible for benefits.

Community Living Director: Lincoln, IL

We are seeking a Community Living Director in Lincoln, IL. This position will be responsible for creating and maintaining an atmosphere of quality, independence, fun, community and person-centered services for individuals living in a Community Integrated Living Arrangement (CILA).

Responsibilities include:

  • Assure each individual receives a person-centered Individual Service Plan (ISP) annually, or more often as needed, and coordinate all choices, services and recommendations identified.
  • Oversee the implementation of the home’s person-centered and active-treatment programs, goals, schedules, and services.
  • Coordinate and oversee individual participation in the community, including special events and memberships.
  • Maintain an active presence within the home to ensure that the operation is meeting the standards of CTF ILLINOIS and meeting the needs and choices of the individuals that reside there.
  • Hire, train, support and manage a team of Direct Care Professionals (DSPs).
  • Coordinate with nursing and other professional staff to ensure the health of each individual is communicated, maintained, and documented.
  • Ensure the overall cleanliness, safety, and maintenance of the home, including coordinating required maintenance, safety inspections and emergency drills.
  • Adhere to each home’s budgetary requirement in the areas of staffing levels, supplies, equipment and utilities.
  • Maintain a working knowledge of current Federal, State, and membership regulations and compliance, DHS, CQL, OIG, HIPPA, OSHA, labor laws, as well as other professional standards, and ensure regulatory compliance.
  • Respond to pager and/or cell phone as scheduled.

Qualifications:

  • BA/BS degree in a human services field
  • Qualified as a QIDP
  • Minimum 1 years’ experience with individuals with developmental and intellectual disabilities
  • Valid DL and acceptable driving record is required
  • Previous Management / QIDP experience preferred

Craft Instructor: Charleston, IL

Full-Time instructor needed to develop and implement craft instruction and programming for individuals with developmental disabilities by selecting and using materials and interventions appropriate to individuals’ needs, and by designing activities to achieve therapeutic and vocational goals and objectives as directed by state and federal regulations and in accordance with the mission of our organization.

Comprehensive knowledge and experience in multiple techniques, trends, production and resale of crafts required. Such experience may have been acquired as a volunteer, through formal schooling, or prior work experience, and demonstrated by a portfolio of work. Experience and demonstrated ability to teach others in production and sale of crafts.

Minimum high school diploma or GED required. Bachelor’s degree preferred. DSP training preferred, though not required. Experience working with individuals with developmental disabilities preferred. Working knowledge of Microsoft Office required, with demonstrated proficiency in Excel preferred.

Requires successful completion of criminal background check. Requires valid driver’s license with satisfactory driving record.

Fine Arts Director: Tinley Park, IL

CTF ILLINOIS is seeking a Fine Arts Director that will be responsible for creating and implementing a multi-modal Arts curriculum that includes a variety of art forms, including drama, music, photography, fine and applied arts, such as painting and sculpting, and design, with both an onsite and community focus.

Responsibilities include:

  • Promote the mission and vision of CTF ILLINOIS through Arts integration
  • Assist administration in creating a culture focused on the Arts
  • Create an exciting and engaging learning environment that values the Arts to all types of learners at levels that work best for them
  • Develop each person’s interest, ability, and talent for creative expression
  • Work directly with leadership and management to integrate Arts into curriculum throughout CTF ILLINOIS
  • Be responsible for the general “look” of all arts programs and facilities
  • Promote the Arts through special events and/or performances, including creating, designing and producing one to two events a year to showcase the wide variety of creative opportunities, new skills and talents
  • Work directly with Direct service professionals to implement Arts curriculum
  • Responsible for operating budget, including the purchase, management and maintaining of curriculum and program materials as needed
  • Create and sustain contacts in the arts community
  • Integrate people supported by CTF ILLINOIS into the local arts community, including theaters, art shows, and musical groups
  • Secure volunteers with an arts background to help with the arts part of the program, different art mediums, music teachers, etc

Secondary Responsibilities:

  • Keep leadership informed and updated as needed
  • Provide weekly and/or monthly report
  • Attend meetings, community events, etc.

Required Experience, Education, and/or Training:

  • Bachelor's Degree (B.A.) from a four-year college or university in a art education and/or related field, or equivalent experience
  • Working knowledge of Microsoft Office (Excel, Word, Outlook,etc.), and/or other PC applications
  • Possess valid drivers license with acceptable driving record per company requirements
  • Demonstrated knowledge of the Arts including Fine arts, theatre, dance, music, etc.
  • Demonstrated experience teaching one of the arts
  • Demonstrated experience in cultural promotion of the arts

Additional Experience, Education, or Training Preferred:

  • Prefer previous experience with individuals with developmental disabilities
  • Highly creative and flexible
  • Works well as part of a team
  • Must be able to work successfully with a wide range of people who have varied art interests and backgrounds
  • Ability to communicate and engage other program staff in the collaborative projects that are the core of the arts experience
