Professional person needed to supervise day program site for individuals with developmental disabilities. Monitor the overall operation of the site by coordinating and monitoring direct care staff (DSPs) and ensuring the safe operation of the day program. Interview, hire, train, mentor, support, and supervise direct care staff. Maintain a working knowledge of State and Federal regulations and professional standards, and oversee compliance of these regulations and standards. Complete documentation including, but not limited to, monthly attendance reports, conduct safety inspections, maintain equipment, and order supplies. 

Requirements:

  • Minimum of one-year experience working with developmentally disabled population
  • HS Diploma/GED
  • Current valid driver's license with acceptable driving record
  • Previous supervisory experience

QIDP - Olney, IL

Professional person needed to advocate for and support individuals with developmental disabilities by coordinating and monitoring the overall plan of care as directed by state and federal regulations. Develop and implement goals for each individual receiving services and ensure services are ongoing and consistent with goals established. 

  • Bachelor's degree in Human Services or related field
  • Minimum one-year experience working with DD population required
  • Previous QIDP certification preferred, but not required
  • Must meet qualifications for QIDP certification
  • Current valid Driver's License with acceptable driving record required

Evening Maintenance Janitor needed to perform routine janitorial duties and maintenance of buildings. Follow established safety rules, policies, and procedures. Full-time position, evening shift. $18,449.60 plus benefits.

Full-Time Health Care Coordinator - Charleston, IL

Full-time Health Care Coordinator needed to schedule and coordinate medical services for adults with developmental disabilities. Will transport/accompany individuals on appointments, maintain documentation under supervision of nursing staff.

  • Full-time Position; 40 hours per week
  • Coordinate and schedule medical services and appointments for individuals receiving services
  • Check monthly for appointments (i.e. medical, vision, dental, neurological, etc.) that are due to be scheduled for residents
  • Schedule and confirm appointments, coordinate transportation needs, and accompany individuals on all appointments
  • Ensure that a medical synopsis form (completed by the nurse) is taken to appointments as appropriate, scan all paperwork to the team after appointment is completed
  • Ensure physician signature is obtained on Quarterly Med Reviews
  • Assure the delivery of each individual's financial assistance for the payment of cost of care, food stamp eligibility, and medical services by coordinating benefits with the Illinois Department of Human Services
  • Ensure all individuals have Medicaid cards

Requirements:

  • High school diploma or GED
  • Must be at least 18 years of age
  • Prefer at least one year experience working with individuals with developmental disabilities. 
  • Ability to attend training and become an approved driver for the Medicar program
  • Ability to read and interpret documents such as policies and procedures, individual programs as described in the program plan and medication orders
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers
  • Ability to solve practical problems and deal with a variety of situations
  • Ability to interpret and complete agency forms
