Part-time Health Care Coordinator - Charleston, IL

Part-time Health Care Coordinator needed to schedule and coordinate medical services for adults with developmental disabilities. Will transport/accompany individuals on appointments, maintain documentation under supervision of nursing staff.

  • Part-time, approx. 24 hours per week, with ability to work DSP shifts if desired
  • Coordinate and schedule medical services and appointments for individuals receiving services
  • Check monthly for appointments (i.e. medical, vision, dental, neurological, etc.) that are due to be scheduled for residents
  • Schedule and confirm appointments, coordinate transportation needs, and accompany individuals on all appointments
  • Ensure that a medical synopsis form (completed by the nurse) is taken to appointments as appropriate, scan all paperwork to the team after appointment is completed
  • Ensure physician signature is obtained on Quarterly Med Reviews
  • Assure the delivery of each individual's financial assistance for the payment of cost of care, food stamp eligibility, and medical services by coordinating benefits with the Illinois Department of Human Services
  • Ensure all individuals have Medicaid cards


  • High school diploma or GED
  • Must be at least 18 years of age
  • Prefer at least one year experience working with individuals with developmental disabilities. 
  • Ability to attend training and become an approved driver for the Medicar program
  • Ability to read and interpret documents such as policies and procedures, individual programs as described in the program plan and medication orders
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers
  • Ability to solve practical problems and deal with a variety of situations
  • Ability to interpret and complete agency forms
